- We offer both a downloadable form or an online version for your employee to complete.
- Our physician will review the questionnaire to decide to either clear the employee without restrictions or schedule a follow-up for further assessment.
- If a follow up is needed, the physician will schedule a virtual visit to address any questions that arose from the questionnaire.
- The goal is to address any concerns and ensure the employees well-being and safety.
- Once a determination has been made, documentation will then be provided stating the level of clearance for respirator use.
- This along with the pulmonary function test is used to medically clear an employee to participate in your company's respirator protection program.
